This letter may be used by UNT employees and students to request permission of copyright holders to use copyright protected materials. The individual making the request should download the form letter and fill in the appropriate form field sections with specific details regarding the planned use:

• date
• copyright owner's name, address, etc.
• role of the person requesting permission (i.e., student, instructor, or staff member)
• title of book, article, or webpage heading along with the amount of material to be used
• description of how the material will be used
• information regarding attribution that will be disclosed to readers
• signature
